Your funeral
director can provide you with a shipping container, if necessary.
2.
After contacting
Bay Watch Cruises and faxing the Authorization
Form, please enclose the following with the cremated remains
of your loved one.
a) The
original Authorization
Form
b) This
Services Order Form
c) Any
original disposition documents (such as a burial permit)
d) A
copy of the Death Certificate
e) Your
check or money order payable to Bay Watch Cruises (please contact
us for instructions to pay by credit card)
3.
Mail these
items by U.S. Postal Service only to:
