authorize Bay Watch Cruises to scatter the cremated ashes of _____________________
(Deceased) at sea, by vessel, in the Pacific Ocean, off the Coast
of San Diego County. I certify that I have full legal right and
authority as
Primary Next of Kin, and know of no living person who has
superior right under state law, to authorize the disposition of
the cremated ashes of the Deceased. I understand that scattering
the cremated ashes will occur within 60 days of receipt and is
subject to the terms and conditions set forth in this (1) Authorization
Form, (2) Services
Order Form, and (3) any applicable Federal, State, or local
laws and regulations. I
understand the obligation of Bay Watch Cruises is limited to scattering
of cremated ashes as directed in this Authorization Form and Services
Order Form. I agree to hold harmless and release Bay Watch
Cruises from any and all liability, loss, damage or causes of
action in connection with the disposition of the cremated ashes
of the Deceased. Bay Watch Cruises is not responsible for any
damage or loss of cremated ashes while being shipped.
